Report an Absence

  • If your child will be absent from school, please CLICK HERE to report an absence to the data manager.  All absences and tardies are recorded as Unexcused until the appropriate documentation is received AND it meets the UCPS Attendance Policy (BOE Policy 4-1a). 

    Excused Absences with Required Documentation 

    • Illness or Injury – Must be submitted within 3 days of the absence via the online form link above or a handwritten note to the teacher that includes:
      • Child's Legal Name (first and last name)
      • Child's Teacher's Name
      • Date of Absence
      • Reason for Absence
    • Death in Immediate Family – Must be submitted within 3 days of the absence.  For the purpose of this regulation, the immediate family of a student includes a parent, grandparent, brother or sister.  Student is allowed 3 days of excused absences.
    • Medical or Dental Appointment – Medical or Dental office note
    • Court or Administrative Proceedings – Documentation from the court or appointment
    • Religious Observances – Student is allowed 2 days each academic semester with prior written approval from the principal
    • Educational Opportunity – Student must complete the Educational Opportunity Request Form and have it approved by the principal prior to the educational opportunity
    • Quarantine - Hospital, doctor or Health Department note
    • Military Deployment – Student attending ceremonies related to military deployment of family members

     Unexcused Absences (to include but not limited to):

    • Missed bus or transportation issue
    • Out of town/vacation
    • Anytime a student is not in school without a valid note

    Excused Tardies/Early Dismissals with Required Documentation (to include but not limited to):

    • Illness on the part of the student - Written parent note submitted within 3 days of absence or Nurse Sent Home
    • Doctor or Dentist Appointment - Medical or Dental office note
    • Late Bus Arrival

    Unexcused Tardies/Early Dismissal (to include but not limited to):

    • Illness on the part of the student without a note from the parent
    • Oversleeping, traffic, carpool trouble or other “personal reasons”
    • Out of town/vacation
    • Missed bus
