Meningococcal Conjugate (MCV4) Vaccine
Attention Parents of Rising Seniors! North Carolina law requires all students to receive a second dose of the Meningococcal Conjugate (MCV4) vaccine before entering 12th grade. We encourage you to schedule this important vaccination over summer break to ensure your student is ready for senior year. Once completed, please submit an updated immunization record to our school nurse as soon as possible. Thank you for helping us keep our students healthy and in compliance with state requirements!

You will be able to submit screenshots of doctor notes, parent notes, etc. when you submit. First semester, using the form will be a choice. Second semester, we will be exclusively using the form.
